Sample Submissions
- All samples submitted to the laboratory should be accompanied by a completed Sample Submission Form. Kindly use our latest version (2015) from the 1st of March 2015. For a copy of the Form in Word (with Text Boxes) please mail reception@fdalab.co.za.
Sample Preparation, Labelling and Packaging
- Handle samples with gloves and tweezers where applicable, to avoid cross-contamination.
- Pack each sample so that it will not be contaminated by any other samples.
- Make sure to properly and clearly label the sample(s), indicating the name and identifying what you want analysed.
- Pack the samples so that they will not break in transit.
- Submit a signed Sample Registration form with the samples that describe the work you want done. A copy of the form may also be e-mailed or faxed to us upon despatch of a sample..

Our laboratory is open from 8 am to 5 pm on weekdays only. By special arrangement, samples can be received on Saturday mornings.
- Use any trusted courier of your choice.
- If the sample needs to remain frozen or refrigerated, please ensure it is appropriately packed for the duration of the transfer.
- Supply courier with the correct physical address and contact numbers.
- If necessary, advise us upon despatch of a sample, and estimated arrival time.
